Clients may review or change their information provided to us by contacting our agency.

Preferred Care at Home collects and uses your personal information upon your request to deliver the services or information you have requested. Personal Health Record
Preferred Care at Home provides a free blank Personal Health Record (PHR) tool which may contain personally identifiable information, that is solely up to the individual to download, fill out and maintain.

Preferred Care at Home will encourage the use and maintenance of a PHR, but takes no responsibility for its content, validity or whether the PHR has been maintained. Preferred Care at Home is purely supplying a free tool for the client to easily gather important healthcare information.

The Personal Health Record can be uploaded and attached to the client’s chart in the Home Care Software only with the client’s or legal representative’s permission. The secure Client Login, on the homepage of our website only links access to those with the appropriate authorization.

Family members and primary physicians along with other healthcare providers may access this information with a username and password only if permission is granted by the client or his or her legal representatives. The PHR information is not housed on Preferred Care at Home’s server and is protected by the third party provider of the Home Care software, who provides the electronic security and firewall.

Preferred Care at Home may use your personally identifiable information to inform you of other services available from us. Preferred Care at Home may also contact you via surveys to conduct research about your opinion of current services or of potential new services that may be offered. Preferred Care at Home also may disclose information to entities and persons affiliated with Preferred Care at Home; vendors; clients; employees; independent contractors; agencies; referral sources; background check and other similar agencies; and banks, credit card or other financial service companies with consent.

We collect non-personally identifiable information through the use of General Tracking Information and Click Path Tracking (cookie technology) described below.

Preferred Care at Home does not sell, rent or lease its customer lists to third parties.

Preferred Care at Home may keep track of the websites and pages our customers visit within, in order to determine what services are the most popular. This data is used to deliver customized content to customers whose behavior indicates that they are interested in a particular subject area. Additionally Preferred Care at Home may use your IP address and other information to compile website user statistics and administer its website.
