Homemaker Care

As we age, a big part of happiness depends on being comfortable and cared for at home. Home is where memories have been made, and are still being created.

When caring for the home becomes a burden the unwelcome thought of moving strikes.

Should you live in a home you’re unhappy with—one that’s getting cluttered and dusty because you can’t do the chores like you used to? Should you learn to cope with eating less because it’s difficult to shop and cook? Should you give up your pet because you can’t care for it? Should you put up with wrinkled or dirty clothes?

Transition Care

You’ve probably seen it with loved ones. An elderly person checks out of a hospital or nursing home only to be readmitted within a few months.

During the transition, medications weren’t taken, doctor appointments weren’t made and chronic illnesses weren’t cared for. The downward spiral seems unstoppable, the medical bills never-ending. And you’re at a loss for what to do.

At least one in five people with Medicare are readmitted to the hospital within one month of discharge.

Hospital-to-Home Transition Care

Transition care—changing a downhill slope to a recovery triumph

You’ve probably seen it with loved ones. An elderly person checks out of a hospital or nursing home only to be readmitted within a few months. During the transition, medications weren’t taken, doctor appointments weren’t made and chronic illnesses weren’t cared for. The downward spiral seems unstoppable, the medical bills never-ending. And you’re at a loss for what to do.

At least one in five people with Medicare are readmitted to the hospital within one month of discharge.

But what if hospital care were different? Instead of sending elderly people home alone with confusing instructions and to-do lists, what if we also sent a coach—a trained expert to guide, encourage and empower? What if that transition care service were affordable?

At Preferred Care at Home, this isn’t a pipe dream. We’re making it a reality through our innovative Smooth-Transition Care program.

What Is Smooth-Transition Care®?

A transition from a hospital or nursing home can be confusing. You get rehabilitation instructions, prescriptions to fill, follow-up appointments to make. When you get home, you don’t just have to build up your strength, but you’re supposed to remember all these other things—and have the energy to do them. For elderly people, recovery can be even more overwhelming because they’re often alone.

With our Smooth-Transition Care® program, we’ve addressed each of these problems and developed a solution that helps put patients at ease so they can focus on getting better.

As a Smooth-Transition Care® client, you’ll receive three tools to help you stay out of the hospital:

1.  A personal, specially trained coach

Your coach will meet you at the hospital, nursing home or other health facility and guide you through the transition process. After your discharge, your coach will meet with you regularly, through phone calls or visits, and make sure you have what you need. For example, your coach may schedule transportation to follow-up appointments, remind you about prescriptions and medication regimens, and make sure your home is safe. Your coach will also provide a listening ear and encouraging words.

2.  A Personal Health Record.

This in-depth, 15-piece tool kit stores the information you need to transition optimally, including medication instructions, appointment dates and test results. Your coach will encourage you to use this tool. The record also includes an easy-to-read symptoms chart that shows when you can wait and see your doctor and when you should call 911. You can view the Personal Health Record and even download the pages for free.

3.  An easily accessible online health record.

No need to tote the Personal Health Record with you to every doctor appointment. As a Smooth-Transition Care® client, you’ll have exclusive access to your own online health record. Upon your request, your coach will add this information to your secure, password-protected database. You can share it with health care providers and family members who need up-to-date information.

End-of-Life Care

Tender, loving comfort and support

As you wind down in life, you may need someone in your home around-the-clock for total peace of mind. The support our end-of-life care team delivers is a constant source of comfort.

Empathy

At Preferred Care at Home, we partner with hospices to care for people as they approach life’s final transition. Our caregivers are specially trained in end-of-life care. Depending on your needs, they may, for example, prepare easy-to-eat foods, read to you, and watch for discomfort and summon health-care providers if needed.

With end-of-life care, we help people who are dying remain in the peace and comfort of their beloved home. If a hospice facility is required, however, we also provide loving services there.

After the person dies, the caregiver will also take care of the immediate tasks that need to be done:

  • Notifying the appropriate people
  • Providing privacy and dignity

End-of-life care is also available on a short-term respite basis. Our trained, compassionate aides give the family caregiver time to rejuvenate or accomplish tasks that must be seen to. However long you need us, we’ll be there.

"We are humbled to serve our community and their loved ones."

Fred & Karla Gushue, Taylor & Lindsay Gushue

Fred and Karla Gushue raised their children to keep faith, family, and teamwork above everything else. And those three truths have led their family of five to achieve great things even before they received their calling to help families through homecare.

Fred oversaw several successful restaurants, became an ordained pastor, and worked for the WinShape Foundation, one of the largest charitable organizations in America. Their son Taylor was a gifted baseball player for the University of Florida, and eventually the MLB, where he played for the Pittsburgh Pirates, Washington Nationals, and the Chicago Cubs.

Through all their successes in their diverse paths, family was always their North Star, which is how homecare became part of their journey, present and future. There is a great need for homecare in Brevard, and Fred, Karla, and Taylor know they can make a great difference for those families in need.

As the owners of Preferred Care at Home of Brevard, the Gushue team hopes to bring their boutique family-centric approach to those needing care in Brevard. If you are looking for a dependable team, guided by faith, reach out to Preferred Care at Home of Brevard today!

Fred Gushue

Fred was born in Brooklyn, NY and grew up in Deerfield Beach, Florida. He and Karla married after college and raised their 3 children in Boca Raton surrounded by extended family. Fred’s early career was spent as a manager and director overseeing several successful restaurants. One day his father (who was an elder at the church he and his family attended) asked him, “Fred, what are you doing to serve the Lord?” This question inspired him to pursue finding a way to help at church using his gifts and knowledge. He noticed a need within the church to have someone who could help with food services. Since this was an area he had become an expert in, he offered to help. That quickly turned into a full time job as he went on staff at Calvary Chapel, Ft. Lauderdale. Over the years that he spent working there, he served in various leadership roles such as director of food services, director of Calvary house, director of missions… and also joined the pastoral team when he was ordained in 2012. A few years later he went on to work for the WinShape Foundation in Rome, Georgia. WinShape is a charitable organization founded by Truett Cathy of Chick-fil-A. Fred spent 4 years with the foundation serving on the leadership team. Fred and Karla moved back to Florida to be near family after becoming grandparents. His heart for meeting the needs of others led him to Preferred Care at Home. Fred’s own dad was blessed by the assistance of a home health care professional in his time of need. He saw how having a helper in the home allowed his father to retain some independence, confidence and dignity later in life. He also saw the added benefit of how it lifted some of the caretaking responsibilities from his mom and sisters allowing those important relationships to maintain a healthy focus and role during a difficult time. Still inspired by his father’s question and the legacy his father left his family, Fred’s desire is to leave a legacy of love, faith and service much like his beloved father’s example.

Taylor Gushue

Taylor was born and raised in Boca Raton, Florida. As a gifted athlete, he spent his youth playing various sports but later focused on baseball and earned a scholarship to the University of Florida. He enjoyed a successful college career there as the starting catcher, playing in the college world series and being nominated for the Johnny Bench award. He was drafted by the Pittsburgh Pirates in 2014 and went on to play for the Washington Nationals and the Chicago Cubs where he made his big league debut. He also had the honor of being on Team USA while competing in Mexico and Japan for the prequalifying games of the 2020 Olympics. Becoming a professional athlete fulfilled a lifetime goal of personal achievement and taught him the valuable lessons of perseverance, hard work and team work. No one wins a World Series or a gold medal without the helping hands of those who invest in your life, coach you, and encourage you to do what you need to do and believe that you can accomplish it. Taylor and his wife Lindsey are dedicated to their new PCAH team. Taylor brings his life lessons from his baseball career to Preferred Care at Home. Taylor’s goal is to provide a winning team that will be the helping hands and encouragement for our most valued players, our senior community!
