MarQuietta & Mareio Lawson
MarQuietta realized that there was a need for home care services when Alabama’s seniors found themselves being hospitalized repeatedly. A knowledge deficit of their disease process played a role in this, but the fear of losing their independence and the thought of being a burden to their loved ones played a bigger role. Time is so precious and spending it with our seniors in the comfort of their own home, while receiving affordable, compassionate and reliable care is very special and rewarding. This is why MarQuietta and her husband Mareio took action.
Alabama residents and owners of Preferred Care at Home of South Alabama, MarQuietta and Mareio are a husband and wife team who have three beautiful daughters. Family is central to who they are and the care that they provide.
As owner and administrator, MarQuietta brings much experience to her role. She started her medical journey as a Certified Nurse Assistant. It all began with her love for helping people. This led her to becoming a Registered Nurse and working in the medical field for over 20 years. Throughout the years she has served as a Nursing Supervisor for Neurosurgery and Neurology, Medical-Surgical, Case Management, Internal Medicine, and Home Health experience. Her faith, passion, and commitment to helping people earned her the Daisy Award, which recognizes extraordinary nurses. This recognition is one of the most honorable and overwhelming moments in her nursing career and would not have been made possible without the support of her husband Mareio, her family and friends, and her faith and belief in God. Family, friends, and faith– it is what motivates and inspires her each and every day.
As a medical professional and the owner of Preferred Care at Home servicing Mobile, Baldwin, Monroe County and beyond, MarQuietta knows that asking for help is not easy and losing your independence may be your greatest fear. How Can Smooth-Transition Care® Help With Your Hospital-to-Home Transition?
Going home from the hospital is not just about moving your stuff; you need a plan. Designed to empower patients and their families throughout this vulnerable time, hospital-to-home care is a comprehensive and proactive approach to transition care. We not only come alongside you; we walk with you on your journey to recovery, providing you with guidance, support, and the tools you need to transition with long-lasting success and wellness. It is not only about reducing the likelihood of returning to your hospital bed; it is about supporting your safe, confident, and comfortable reintegration back into everyday life.
At the heart of the transition care program is a multi-pronged strategy to tackle the top issues patients commonly encounter after leaving the hospital:
- Transition Coaching: Realizing how daunting post-hospital care can be, we offer a dedicated transition coach. A trained and seasoned professional who takes on the role of a personal guide, their presence and support start from the day of leaving the hospital and progress for the next critical few weeks at home. The coach not only helps patients practically — they help patients empower themselves, motivate themselves, and generate a sense of control/systematic improvements over their health journey. Having someone with this specialized training and knowledge can help provide a vital lifeline in a patient’s overall confidence and adherence to their recovery plan.
- Good Communication: Communication is key during the transition phase. Our transition coach serves to help ensure that the hospital staff has clear communication with the patient and their family. This helps to make sure that everyone has the most accurate information about the care plan, medication instructions, and follow-up appointments. On top of that, the coach helps create organized documentation for important info, like medication schedules and appointment details, reducing confusion and minimizing potential mistakes. Having this information readily available supports a healthier and easier transition back home.
- Encouragement and Accountability: It can be difficult to keep momentum and stick to the recovery plan once discharged. Our coach supports, motivates, and checks whether things are being followed as scheduled and practical things get done the right way. The coach regularly checks in to track progress, address any developing issues, and assist with transportation to follow-up medical appointments and medication compliance. They are also crucial for monitoring risks for depression in recovery, which can often creep up, and facilitating therapy if needed.
- Personal Health Record: To further support patients and their families, our transition care program includes a toolkit. Sick patients can use this invaluable resource to manage crucial health information. There are sections for medication lists, appointment schedules, contact information for healthcare providers, and other essential details. Patients are provided a unique access code that allows family members, caregivers, and healthcare providers to see the most current information anytime they need it, anywhere they are. Having access to this information greatly improves coordination of care and helps reduce the risk of miscommunication (and the possibility of readmission).
Smooth transition care is an integrated program delivered by our hospital-to-home care that promotes a whole-person, whole-family approach to care by incorporating four components of transition care.
Hospital-to-Home Transition Care Services in Mobile, AL
The needs of those coming home from the hospital are not the same. That is why we have a holistic range of hospital-to-homecare services, all of which can be customized to suit both individual lives as well as the needs of recovery. We aim to offer the right amount of care so that individuals can seamlessly transition home, enabling a return to independent living and a nurturing environment. We offer the following services:
- Medication Reminders: One of the most important things about post-hospital care is following your prescribed medications. We have trained caretakers who provide medication reminders to patients at the right times and in the right doses. This essential service reduces the likelihood of medication errors and encourages effective healing while also averting probable complications that can result in the need for readmission.
- Transportation: Follow-up appointments with doctors and therapists are critical for a successful recovery. But transportation is often a huge hurdle. That’s why our hospital-to-home care services include safe and reliable transportation to these critical appointments and even to therapy sessions, pharmacies, and any other necessary outing. This allows patients to receive the continuing medical care they require without additional stress or logistical burdens.
- Homemaking: What awaits at home are household chores, which can be a considerable transition during the recovery period. Our caretakers can help with light housekeeping duties, including organization, laundry, and cooking. By creating a clean, organized, and comfortable living environment, we decrease the overhead of anxiety and allow the patients to focus on their healing. Nutrient-rich, home-cooked meals, made according to one’s dietary needs, also play an important role in the healing process.
- Personal Care: In some cases, some individuals require help with personal hygiene tasks such as bathing, getting dressed, and personal grooming. We recognize the sensitivity surrounding these needs and are focused on allowing individuals to continue with their independence and dignity while also being able to provide their comfort and safety.
- Comprehensive Services: As some people experience a medical condition that requires special care, such as dementia and Parkinson’s disease, our services provide tailored support for those who need specialized medical care, chronic care, etc. Our caregivers receive care for these special needs, so our caregivers are competently trained to care for them in the hospital-to-home transition and beyond.
What is Hospital-to-Home Transition Care?
Transition care, more commonly known as hospital-to-home care, is a holistic and person-centered care that helps people transition from hospital stays or discharge from other medical facilities into their homes. It’s not just about offering physical support — it involves a holistic array of transition care services that are carefully crafted to meet the diverse needs of those on the mend. Some common services include high-level medication management to provide the right medications in the right doses at the right time to prevent poor reactions; private and/or manual transportation to appointments and therapies; personal care to ensure a patient’s dignity; ambulation to keep them mobile; and sometimes, even light housekeeping to take care of daily living activities. Hospital-to-home care primarily aims at the goal of reducing the risk of complications or the need for hospital readmissions and enabling individuals to recuperate from illness and injury to restore their strength, independence, and general quality of life in the comfort of their homes. You need to get to the middle between an institution and a home for an intern and be supported the whole way through the recovery process.
