Preferred Care at Home was founded in 1984 by Lois Whitesell, a registered nurse, along with her daughter Jody Guerrieri and son-in-law Frank Guerrieri, in response to something they saw clearly: older people simply wanted to stay home. Read the story of how it all began.

Jake spent years as a licensed administrator for a skilled nursing facility, watching elderly people leave after a hospital stay or short-term rehabilitation and, too often, return within months. He saw the gap: discharge instructions, prescriptions, follow-up appointments, and not enough support at home to follow through on any of them. That experience is what led him to open this office and why our Smooth-Transition Care program sits at the heart of what we do.
His connection to senior care started long before his professional career, through years of volunteering in healthcare communities as a young person. The care he saw given well, and the care he saw fall short, stayed with him. Both shaped how this office works today.

We visit the home, build a personalized care plan, and match a caregiver by personality and experience, not just availability. Every caregiver has passed our 7-step screening process before coming to your door.
If the match isn’t right, we find another. That commitment to continuity is not a policy we list on a website. It is how Jake runs every client relationship. Services are available from one hour a day up to live-in care for seniors who need around-the-clock support at home. Once care begins, our Transparency Room portal gives authorized family members real-time access to caregiver schedules, daily notes, and visit confirmations from anywhere. There is no long-term commitment required in most cases.
Preferred Care at Home of San Antonio North holds Texas license #023022, issued by the Texas Health and Human Services Commission as a Home and Community Support Services Agency. This credential authorizes our operation in Texas and confirms that we meet the state’s standards for home care services.
