Direct Care Through the VA Community Care Network (CCN)
Preferred Care at Home is proud to collaborate with the VA to ensure that our nation's heroes have access to high-quality, timely care right in their own living rooms.
Through the Community Care Network (CCN), the VA partners with trusted community providers like us to bridge the gap when a VA facility isn't the right fit or is too far away. This program is designed to provide you with a direct link to the specialized support you need, authorized and funded by the VA.

Eligibility & Enrollment:
The veteran schedules an appointment with a physician at the VA. An assessment is preformed to determine eligibility.

Referral to a Case Manager:
Once enrolled, you are assigned a VA Case Manager who evaluates your specific needs —calculating exactly how many hours of care will best support your independence.

Collaborative Care:
We work hand-in-hand with your VA medical team to follow a customized care plan, ensuring that your health and safety are monitored by people who truly care.

The Preferred Care Match:
Upon approval you will be assigned a caregiving agency, but you can request to use us! We then step in to match you with a caregiver who understands your history and respects your home.

The Smooth-Transition Care® Program:
If you are being discharged from a VA hospital, our Transition Coaches ensure a safe return home, coordinating everything from medication reminders to follow-up appointments to prevent readmission.
24-Hour & Live-In Care:
For veterans who require around-the-clock support, we provide consistent, compassionate caregivers who become like family, avoiding the "revolving door" of strangers.
