What Is Smooth-Transition Care®?

A transition from a hospital or nursing home can be confusing. You get rehabilitation instructions, prescriptions to fill, follow-up appointments to make. When you get home, you don’t just have to build up your strength, but you’re supposed to remember all these other things—and have the energy to do them. For elderly people, recovery can be even more overwhelming because they’re often alone.

With our Smooth-Transition Care® program, we’ve addressed each of these problems and developed a solution that helps put patients at ease so they can focus on getting better.

As a Smooth-Transition Care® client, you’ll receive three tools to help you stay out of the hospital:

1.  A personal, specially trained coach

Your coach will meet you at the hospital, nursing home or other health facility and guide you through the transition process. After your discharge, your coach will meet with you regularly, through phone calls or visits, and make sure you have what you need. For example, your coach may schedule transportation to follow-up appointments, remind you about prescriptions and medication regimens, and make sure your home is safe. Your coach will also provide a listening ear and encouraging words.

2.  A Personal Health Record.

This in-depth, 15-piece tool kit stores the information you need to transition optimally, including medication instructions, appointment dates and test results. Your coach will encourage you to use this tool. The record also includes an easy-to-read symptoms chart that shows when you can wait and see your doctor and when you should call 911. You can view the Personal Health Record and even download the pages for free.

3.  An easily accessible online health record.

No need to tote the Personal Health Record with you to every doctor appointment. As a Smooth-Transition Care® client, you’ll have exclusive access to your own online health record. Upon your request, your coach will add this information to your secure, password-protected database. You can share it with health care providers and family members who need up-to-date information.

Suzette Gondera, RN

Suzette Gondera is one of the proud owners of Preferred Care at Home of Coastal Volusia. She proudly serves her community by providing exceptional in-home senior care.

Suzette has dedicated her life to healthcare and family. In 1980, Suzette began her career as a Registered Nurse with Florida Hospital (now called Advent Health) and has continued there for nearly 40 years. Her health care experience spans from the Intensive Care Unit, the Post Anesthesia Care Unit, Endoscopy, Outpatient surgery to Home Health. In the last 20 years, her focus shifted to Pain Management where she cared mostly for elderly patients suffering from chronic pain.

Suzette’s experience in her professional career has shaped who she is and her passion for quality care. Additionally, her personal experiences with her grandmother and mother have influenced the expansion of her role into the world of senior home care.

At 105 years old, Suzette’s Grandmother, Leonor, was hospitalized and subsequently moved to a nursing home. This was a huge shift for Leonor as she had spent years dedicated to the care of her own mother who lived to 100 years of age and remained in her home the entire time. Remaining in her home was Leonor’s heart’s desire and when in the nursing home she would ask over and over, “When can I go home?” Unfortunately, she never had the chance to return home.

In both her professional and personal roles, Suzette has seen the desire for seniors to remain in their home. When given the choice, seniors prefer home. Now, Suzette can make these hopes a reality through quality home care services.

For over 44 years, Doug has been by Suzette’s side. While running his own chemical company, Doug and Suzette raised their three kids in Central Florida. Beyond business, Doug established a 100% volunteer natural disaster response team in 2004. Its main purpose is to provide spiritual and physical nourishment after a natural disaster. Hot meals are served to all in need after natural disasters including hurricanes Charley, Wilma, and Katrina and tornado devastation at Lady Lake, Florida. Because of his community effort and response Doug was awarded an honorary Doctorate of Humanities from a local university. Doug has also served as Rotary President and is a current member of the Rotary. Doug’s commitment to the community extends to the most vulnerable–the seniors who need support and care in order to improve the quality of life while safeguarding their dignity and independence.
